Device disconnect while updating to 1511

While trying to install Windows 10 Pro 1511 on a Toshiba laptop using Windows Update:
1: Download was successful
2: Configuration was successful
3: Copying files was successful
4: Setting up drivers, was successful
5: In configuration, connection to the SSD boot device was LOST and could not be recovered.
6: Windows 10 Pro was reinstalled automatically.

What must be done to get past this problem?
