Toolbar icon disappeared and out of function

The toolbar icon disappeared suddenly like below screen, and the remaining icon (included Start) can't left clicked as well.
I have read some forum that adding Users can solve the problem, but unfortunately the Settings is also out of work like the second screen capture, and thus can't add new users, how can I fix the problem?
