PC Disk Running at 100% - Where to Start for a Fix

My PC's SSD has been running at 100% usage for a month. I’ve tried factory resets, reinstalled Windows (with errors), and various commands, but nothing works. The issue persists even on different hard drives, and I’ve checked RAM sticks.

Could it be a hardware problem, possibly with the motherboard? Any suggestions on where to start?
