desktop icons layout messy every time something is connected to usb ports

Since I had to boot on safe mode last week I started to have an issue with icons.
(on safe mode icons are rearranged to fit the limited video resolution)
Every time I insert a device on USB ports the icons are rearranged as on the picture enclosed.
I don´t employ auto arrange , I like to have my own customized layout.
Any suggestion ?
