Why does this kind of thing appear ?
All I want are an inbox and an outbox

And I don't want this "conversation" feature. If I reply to an email, I don't want all the older bits, which I then have to try to delete
All I want is an empty reply box ( with the recipients email put in by the system ), and then I can cut and paste anything old back in

The items you mention are configurable, so that, in most cases, your are not troubled by them.
But, which Email client are you using?
