Output sound keeps automatically reducing to zero

while playing music or watching youtube etc the sound keeps reducing to zero.
i have tryed disabling / enabling the speakers/headphones and DELLU2417H and the sound does come back on.
i am not quite sure which combination of disable / enable works.
i can scroll the mouse wheel to increase the volume but it reverts to zero after anything between 10 secs and 3 minutes
