User Account Control Broken

When you try to run regedit or anything that requires elevated permissions, the UAC comes up but does not display the option to put in your credentials and instead displays the attached.

Anyone know why this is not loading?

This is a custom Windows 10 image on a domain.

Any help is much appreciated.

The image you attached suggests the User attempting to access regedit.exe or other applications requiring elevated status is not currently approved as a member of the Administrative Group.

The actual splash screen you are seeing, albeit it is not displaying the dialog window to enter credentials is the Administrator Approval mode dialog.

You could via Group Policy editor expand the left pane content to:
  • Local Computer Policy > Windows Settings > Security Settings > Local Policies > Security Options
  • Then in the centre pane scroll to bottom and change all Administrator Users to Run in Administrator Approval Mode.

Alternatively, you could manually remove the current User from the Administrator Group, then add that same User back to the Administrator Group to rule out a possible corruption is the Group's membership list,
