It's seems to be a typical prompt for credentials to access the shared resource on a host machine.
You'll need to include the machine name in the top box of the prompt as in
MachineName\UserName
Password
So if the hosting server is named MyServer and there is a User on MyServer with appropriate credentials to access the resource named MyUser and his password is P@$$w0rd then it would look something like

MyServer\MyUser
P@$$w0rd
