Login Checkbox list for kids question

Hello There,
Not sure if this is the right place for it but I hope that either someone can help me or point me in the right direction.

I'm looking for a way to have a sort of checklist for my kids were they have to tick that they have done their Chores and once they have ticked off all the chores they can click on "request access" which should then sent an email to myself and my wife with the list of chores they ticked as done and then for us to authorize access to the Computer/Windows and with the list of ticked off chores we can then do spot checks on if they have actually done them.

Would be great if someone could help me or point me in the right direction.

What you're asking for is a software application that would need to be developed in a computer language which supports a Graphical User Interface (GUI). Microsoft has nothing like this in any version of Windows.
