Why does Windows Defender notify of automatic startup?

I have other protection and have Windows Defender turned off. So, why do I get a notification related to Windows defender that a new auto startup program has been detected, and do I accept this change or reject it?

i don't understand why this is even popping up since Defender is turned off. See screenshot.

Since this is not a Windows generated message (why do you even have that crappy program - Winpatrol that is?), you can do whatever you like. If you accept it, in theory, you should not be bothered with it again...
BTW: Defender is always running in the background even though its not active...

The last part of your answer is what I needed; that Defender is always running in the background even though it is not active. I didn't know if it was supposed to be. Now that I know that I will accept and won't worry about it.
